Cancer Care Coordinators help support GP practices within the Primary Care Network, working within professional and clinical boundaries as part of an established multi-disciplinary team to deliver timely and personalised care for patients.
They identify patients with new cancer diagnoses in order to reach out and offer and support they may require, signposting them to useful information which may benefit them and their family during their difficult journey. They also provide help and support to patients who may be affected by cancer in a non-direct way.
Our Cancer Care Coordinators work across our practices to help patients who are directly and non-directly affected by cancer in their life, providing support and helping to manage their needs. Cancer Care Coordinators can:
work with patients newly diagnosed with cancer and on the cancer register to deliver personalised care
ensure patients receive a Cancer Care review in line with national defined timescales and targets
help patients to manage their needs through answering queries, making and managing appointments
support patients to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing
provide coordination and navigation for patients and their carers across health and care services, working closely with others who can support them
Our Cancer Care Coordinator will run searches to identify patients across our practices with new cancer diagnoses and reach out to them to offer them personalised care and support through their journey with cancer.
